Fees and Rebates
A valid credit/debit card is required at booking to secure all appointments, including bulk-billed consultations. Payment (where applicable) is automatically processed post-consultation via your card on file.
Private Consultations
Initial (50 minutes): $150
Review (30 minutes): $90
Private health insurance rebates may be available, depending on your fund and level of cover. Itemised invoices are provided post-consultation for you to submit directly to your health fund. It is your responsibility to confirm your eligibility with your health fund prior to your appointment.
GP Chronic Condition Management Plan (GPCCMP) Consultations
A valid GPCCMP referral is required to access Medicare rebates. When booking, you can select either a standard (bulk billed) consultation or an extended consultation, depending on your needs.
Standard GPCCMP Consultations: Initial and Review (up to 20 minutes)
Initial and review consultations are bulk billed, with no out-of-pocket cost, where all of the following conditions are met:
A valid GPCCMP referral is provided
Medicare eligibility is confirmed prior to claiming
Assignment of Benefit consent is obtained in writing or verbally in accordance with Medicare requirements
Where bulk billing applies, claims are processed via Assignment of Benefit and the Medicare rebate is paid directly to the practitioner as full payment for the service. If Assignment of Benefit consent is not provided, or if Medicare eligibility or referral requirements are not met, the applicable private fee will apply.
Extended GPCCMP Consultations
For complex assessments or multiple concerns:
Initial (approx. 50 minutes): $150
Medicare rebate (current): $61.80
Out-of-pocket cost: $88.20
Review (approx. 30 minutes): $90
Medicare rebate (current): $61.80
Out-of-pocket cost: $28.20
You will be charged the full consultation fee at the time of your appointment. For eligible extended consultations, we will process your Medicare claim on your behalf after the consultation, and the rebate will be paid by Medicare directly into your nominated bank account where applicable. If your referral is invalid, your Medicare claim is unsuccessful, or eligibility criteria are not met, the full private fee will apply.
Cancellation, Non-Attendance & Payment Policy
A valid card is required at the time of booking.
Payment will be processed after your consultation where applicable.
Appointments cancelled or rescheduled with at least 24 hours’ notice will not incur a fee.
Appointments cancelled with less than 24 hours’ notice will incur a cancellation fee of 50% of the consultation fee, charged to the card on file.
Appointments not attended (non-attendance/no-shows) will incur a fee of 100% of the consultation fee, charged to the card on file.
Cancellation and non-attendance fees are private charges and cannot be claimed from Medicare or private health funds.
Outstanding Payments
If a cancellation fee or consultation fee is not successfully processed, an invoice will be issued and payment will be required within 7 days.
If payment is not received within this timeframe, Living Well Nutrition reserves the right to:
Suspend future bookings until outstanding payment is received
Engage a third-party debt collection agency to recover outstanding fees
Recover any reasonable costs incurred in the collection process
